Can I keep my existing Support Services if I change Home Care Provider?
At Home Care Assistance North Coast, we value relationships as much as you do, so if you are changing Home Care providers, but don’t want to lose the relationships you have with your current physiotherapist, podiatrist, or other support services, we can ensure they continue providing the same services to you even after you switch over.
Changing Provider in 3 simple steps
The process of moving your Home Care Package to Home Care Assistance North Coast involves three simple steps. You can manage these steps independently, or we can arrange a visit to work through the process together.
The first step is to inform your current Home Care provider that you wish to change providers. This must be done in writing, but can be done via email, fax or a letter. It is best to provide notice and depending on your agreement, usually 2 – 4 weeks is required.
Call My Aged Care on 1800 200 422. In order to support you, your new Home Care Provider will need a referral code from My Aged Care, so a phone call to them to let them know you are changing providers will enable them to reactivate your Home Care Package referral code for your new provider. Make a note of this referral code for your new Provider.
Call Home Care Assistance North Coast and we will visit your home to set up your new services ensuring a seamless transition from your previous provider. We will need your referral code and the date you will discontinue care with your current provider to arrange our visit.
