The transition from hospital back home can often involve a lot of change for an individual, leading to increased vulnerability.  Day to day routines, medications, diet, and exercise routines can all be impacted and require attention and modification. The first 72 hours can be a critical time and if unsupported, can lead to an increased risk of re-admission to hospital.

This is where Home Care Assistance North Coast can help.  Our specially trained Care Workers can provide transport home from hospital, and once home, provide monitoring, reminders and emotional support to help reduce stress and promote a successful recovery.

Rachael, who is a wonderful caring and helpful manager, is an absolute gem. My every need is taken into consideration and the staff employed to do the actual caring/cleaning are second to none.
